Here is what our behavior chart looks like in class.  (This is an example - None of  our students are on the above chart.)

The students have made a list of rules for our class which are:
      1.  Hands and feet to yourself.
      2.  Mouth closed when someone else is talking.
      3.  Eyes looking at the teacher.
      4.  Hears listening.
      5.  Be nice to others.

Everyone starts out in the Dugout which means the students are ready to learn.

Base Hit - Great Choices
Dugout - Ready to Learn
Strike One - Think About It
Strike Two - Make Better Choices
Strike Three - Parent Contact (This will be a note home telling what behaviors got them there)
